Please read through this entire form as all information is important. Fill out this form to notify our front office if your student(s) in grades K-5 will be absent, tardy, early dismissed or if you are changing their typical dismissal procedure. Please submit a separate form for each student. All absences should be reported by 8:30am in order to avoid the automated absence call. If there is a change in your student's typical dismissal procedure, please be sure to fill out this form as well. It will need to be submitted at least two hours prior to the end of the day. **ALL DISMISSALS/CHANGES MUST BE SUBMITTED AT LEAST TWO HOURS PRIOR TO REQUESTED PICK UP TIME** In order to maximize time on learning, your student will not be waiting for you upon your arrival for dismissal. Students will be called down when you arrive for pick up. Please plan accordingly and have your photo ID ready. If this absence should be medically excused, it will be adjusted in the system upon receipt of a doctor's note. If your child is having a surgical procedure you MUST contact our school nurse to support your child upon their return. It is imperative any/all emergency room visits or changes in healthcare are reported immediately to our school nurse. The request MUST be from a Parent/Guardian email address that is currently in Aspen connected to your student's portal. Any student who misrepresents themselves as their guardian by filling out this form may be disciplined, as this is forgery.
